Youth Outreach Services
About YOSThrough prevention and intervention methods, Rapha Youth Outreach Services (YOS) program offers the support of Clinical Youth Outreach Workers to help youth and families navigate and connect with services and pro-social opportunities. The program is oriented to empower youth and their families to act on their own behalf and build community connections. The program is a provincially funded initiative through the Ontario Black Youth Action Plan.
Prevention & Intervention

One-to-one Support
Through one-to-one support, our Outreach workers’ role is designed to help empower and provide youth, particularly those who are at-risk of significant disadvantage, with individualized programming to meet their unique needs and circumstances.
Connection to Services
Our Outreach Workers work with community resources to coordinate activities and can help youth get connected to services as needs are identified related to:
Employment | Housing | Newcomer/Settlement | Education | Youth Justice System | Mental Health Services and more!
We aim to provide you with the tools to advocate for yourself
We use an anti-black racism and a strengths-based approach to deliver services. By focusing on a client centred approach, we aim to provide you with the tools to advocate for yourself and ensure you are receiving culturally appropriate services by:
- Individualized supports for black youth and their families
- Encouraging opportunities for leadership
- Providing appropriate referrals
- Helping Black youth find their voice and gain confidence in their positive life choices
- Fostering positive self-image and a strong cultural identity
- Encouraging positive and meaningful participation within the community at large
One-To-One Support
Services & Workshops
We provide services and workshops for agencies and the community. Program participants will have the one-to-one support of a Youth Outreach Worker available to assist in areas of identified need.
